Scheduler Notes

IMPORTANT: All Amarillo appointments scheduled for March 19th and later will be seen at the 

Downtown Library (413 SE 4th Ave.)

This program is for households with low-to-moderate income, a household member 55 or older, or disabled or has limited English proficiency. 

Failure to provide all required documents at the time of your appointment may delay preparing your return.

List of Common Docs Required

Failure to provide all required documentation will delay your assistance. We require the following documents:

  • Proof of Identification for Primary Taxpayer and Spouse, if applicable - Driver's License, State-Issued ID, Passport, Military ID, Tribal ID
  • Social Security card(s) or official documentation with social security numbers (or ITIN letters) for you, your spouse and dependents or anyone else on your return.
  • Birth dates for you, your spouse and dependents on tax return
  • Wage and earning statements (Form W-2, W-2G, 1099-R, 1099-Misc, etc.)
  • Interest and Dividend Statements from banks (Form 1099)
  • Health Insurance Exemption Certificate, if received
  • Proof of bank account routing and account numbers for direct deposit such as a blank check or bank statement
  • If filing Married Filing Jointly: both Spouses must be present with proper documentation to do a tax return with a VITA preparer.
  • Total paid for daycare. Daycare provider's tax identifying number, Business Employer ID number, or statement
  • Forms 1095-A, B, and C, Health Coverage Statements
  • All other tax-related documents
